For New Patients
The Elms welcomes new patients and their families to the practice.
Please contact our receptionists on (03) 5367-6662 to book an appointment, or register online as a new patient here.
New patients should present at the clinic 10 minutes before their first appointment to allow time for our reception staff to complete the registration process. All new patients (apart from minors) will be asked to show photo identification. A standard appointment is for 15 minutes, although longer appointments may be requested for patients with complex or multiple conditions.
Please bring your Medicare or Veterans' Affairs Card, and any pension or low-income Health Care Card.
